Using the application form, you can apply for:
- guardianship only
- financial management only
- guardianship and financial management

Points to remember
- Make sure you provide a witness name and contact.
- Send medical and professional reports to the Guardianship Tribunal with an accompanying letter to say that you have made an online application.
- A copy of the completed application form will be seen by the 'person' and 'parties' to the application.
- You should suggest someone as the guardian or financial manager.
