Medical and Other Professional Assessment Reports

Applications to the Guardianship Tribunal for guardianship or financial management orders or reviews of enduring guardianship or enduring powers of attorney should, wherever possible, be supported by the written opinions of persons such as medical practitioners, psychologists or others qualified to give an expert opinion about the condition and capabilities of the person who is the subject of the application.

These reports may be forwarded later if they are not available at the time of application.

Such reports should be based on the expert’s own recent examination or observation of the person and should include at least:

Access to reports

Reports from experts — particularly medical reports — are rarely made available to the parties. This is done only when procedural fairness requires disclosure. If there is any reason why you believe your report should not be made available under any circumstances to any of the parties or to a particular party, please put your request and reasons in a covering letter for the Tribunal to consider.

Reports should be signed and forwarded to the Tribunal by mail or fax.
